Welcome to the Wolf Creek Primary Care Network. We are a collection of doctors and health care professionals working together in the Central Alberta communities of Bentley, Innisfail, Lacombe, Ponoka, Rimbey, and Sylvan Lake to provide you with integrated care for all of your primary health care needs.

 

What is Primary Care

Primary care includes all the services you receive for your basic, everyday health needs. It gives you and your family a home in the health care system, supported by a team who know and understand your unique needs. Primary care is the foundation to building healthier communities and a strong, sustainable health care system. It includes the initial care, treatment, and follow-up of various conditions as well as referrals to the rest of the health system when needed. It also encompasses the promotion of health and wellness, as well as prevention and management of chronic diseases and injuries.

 

What is a Primary Care Network

Primary Care Networks (PCNs) bring local physicians and other health care professionals together to provide comprehensive patient care to Albertans. They are comprised of groups of family physicians working with other health care professionals, such as:

  • Nurse Practitioners
  • Nurses
  • Pharmacists
  • Registered Dietitians
  • Exercise Specialists
  • Social Workers
  • Behavioral Health Consultants
  • Mental Health Professionals
  • And more…

PCNs develop solutions to meet the needs of the local community and their health issues. There are 40 PCNs operating throughout Alberta with more than 3,800 family physicians and more than 1,400 other health practitioners. Patients who regularly visit a family doctor are healthier and live longer. At any given time, there are hundreds of PCN family doctors across Alberta accepting new patients.

PCNs are created through an agreement between physicians and Alberta Health Services. Please note that PCNs are not a governing body to physicians; the College of Physicians & Surgeons of Alberta regulates the practice of medicine in Alberta.

 

About the Wolf Creek Primary Care Network

The Wolf Creek Primary Care Network has been operating in Central Alberta since May of 2008. We provide services for patients in the communities of Bentley, Innisfail, Lacombe, Ponoka, Rimbey, and Sylvan Lake. Within these communities, we support ten different medical clinics and 78 family physicians, serving a patient panel size of over 75,000 people.

Our mission is to deliver the Patient Centered Medical Home through innovative, collaborative, and comprehensive multidisciplinary care.
