I am a Mental Health Therapist within the communities of Innisfail and Sylvan Lake. I am honored to provide a safe space for patients to work through a variety of mental health concerns such as grief/loss, anxiety, depression, stress, adjustment, and relationship concerns. I use a combination of therapeutic methods with a foundation in solution focused therapy. I believe we each have the innate ability to create the life we truly want to live by identifying and embracing our own strengths and resources. As a psychologist I work with individuals who have been referred by their doctors for issues such as anxiety, depression, stress, grief, relationship/family difficulties, or other situational challenges. I have been trained in marital and family therapy, cognitive behavioural therapy, emotion focused therapy, and I take a client centered and non-directive in my approach to therapy. I believe people are highly adaptive and I try to help my clients recognize the underlying needs which their symptoms are attempting to communicate.

My role at the Rimbey Medical Clinic is to provide short-term therapy to those experiencing mild to moderate mental health concerns including anxiety, depression, relationship issues, stress, grief and loss, and adjustment issues. I am a Registered Psychologist with a master’s degree in Marital and Family Therapy. I use a combination of therapeutic methods including cognitive behavioral therapy and narrative/solution focused therapy. I believe that it is an honour and a privilege to work alongside clients to develop new skills and new ways of looking at their world. I believe in the innate resiliency in all of us and work to help people find hope in their situations.

I would love to interview Brene Brown. She is one of my biggest role models in the world of therapy and self development.



Kim, Mental Health Therapist, Sylvan Lake

My role at SFHC consists of providing individual therapy, group therapy, and consultation/education to patients referred for therapy. In my work common concerns for patients is often a combination of relational difficulties, wide range of emotional issues, work stress, grief/loss, and chronic health challenges. I have been working at SFHC for 6 years and have been in my field for 24 years in total. It is an honor to serve the patients in Sylvan, to hear their story, and to witness the change process while working on a multidisciplinary team that focuses on collaboration and continuity of care. Theoretically, I tailor fit each approach to the client and their unique situation, often drawing upon Mindfulness Based Cognitive Therapy, Narrative Therapy, Emotionally Focused Therapy, and Systems Therapy. When clients are provided choice, information, respect, and compassion in a therapy setting anything is possible!

I would choose Dr. Gabor Mate! He is a fascinating physician that became a renowned expert in childhood adversity, trauma, addictions, and relationship between the mind and body.



Roxanna, Mental Health Therapist, Sylvan Lake

Hello, I’m a Mental Health Therapist working out of the Sylvan Lake medical clinics. I have post graduate training in working with anxiety and trauma and use an integrative somatic approach that combines breathing and relaxation exercises with various types of talk therapy, including CBT. I find reward in helping people to see anxiety or reactions to trauma as a very human response common to all of us. We don’t falter because we are defective but because we are human. I believe all of us have the capacity and potential to be healthy and happy and we hold the resources within us to reach our goals. However, there are times in our lives that challenges and traumas overwhelm us and we require support to find our balance again. And that is my work, to support you on your healing path and help you see that you hold everything within you already that you need. I work with individuals, small family groups, and children and youth 8 years and older.

What’s the most embarrassing fashion trend you used to rock?

I don’t think I rocked any fashion trends but I probably should avoid putting out pictures of myself in the 80s.

If you were famous what would you be famous for?

If I was famous, it would be for making people laugh especially if they discover pics of my fashion choices from the 80s.



Wanda, Psychologist, Sylvan Lake and Delburne

As a PCN psychologist I am part of the team at clinics in Sylvan Lake and Delburne. My role is to provide counselling with people who experience mild to moderate anxiety and/or depression, life transitions and changes, relationship stress, and grief and loss. I believe that we are all resourceful and that we can change. I have training and experience in a variety of models including Cognitive Behavioral Therapy, and Solution Focused Therapy and lean toward Art Therapy and mind-body practices. My work brings me something new every day and gives me the opportunity to celebrate with clients in their progress and successes.

I support individuals in becoming more physically active. It is exciting to work with people on physical activity and exercise because there are so many benefits to an individual's physical, mental, and social health with very few, if any, negative side effects. By being physically active an individual can prevent, manage, and treat many chronic health conditions. Exercising to improve fitness increases your energy, improves your mood, and allows you to do the things in life that you want to. I lead exercise programs in Innisfail, do one-on-one consultations, and faciliate various other PCN programs where physical activity plays a role.

I am the Exercise Specialist for Battle River Medical Clinic in Ponoka and Mainstreet Medical Services and Family Medical Associates clinic in Lacombe. I support individuals with cardiovascular, pulmonary, and metabolic diseases and disorders in adopting and maintaining healthy lifestyle behaviors to improve chronic conditions and activities of daily living. I teach group exercises classes and provide one-on-one consultations. You can be referred to me by your PCN doctor or nurse, or you can self-refer by calling 403-783-3399 (Ponoka) or 403-782-1408 (Lacombe) to make an appointment. Appointments can be in-person or over the phone.

As a Registered Nurse I help people make healthy lifestyle choices to prevent or improve chronic disease such as diabetes, obesity, hypertension, COPD, persistent pain, etc. I also help them understand their disease and medications so they feel more in control and able to manage day to day, so they are living  well with a chronic disease. I review medications with patients and help with starting insulins,  adjusting it, or using puffers for those with COPD and how to use them correctly and much, much more.

I am a chronic disease nurse at Sylvan Family Health Center.  My position helps people manage chronic disease. The diseases I  work with are diabetes, COPD, asthma, tobacco cessation and obesity. I work in a team environment with their physician, a pharmacist, mental health, and office staff.

I am a Registered Nurse at Ponoka's Battle River Medical clinic. We have a very active Rural Obstretrical Program here that includes Ponoka and the surrounding area. My primary role is Prenatal Nurse. I work closely with physicians and Ponoka Hospital and Care Centre to ensure a safe and supported prenatal experience. I provide education, support, and prenatal resources. I am also one of three PCN RN team members involved in chronic disease management. We help with diabetes, COPD, smoking cessation, memory testing, medication reconciliations and referrals to the rest of our PCN medical home team as required to promote health in our communities.

I am one of a team of three Registered Nurses at the Battle River Medical Clinic. I work as a Chronic Disease Management Nurse assisting patients to manage their health conditions such as Diabetes, COPD, hypertension and smoking cessation. Another part of my position is to complete frailty screening and cognitive assessments with seniors in our Healthy Seniors Program. I work in collaboration with our other team members including a Dietician, Social Worker, Panel Manager and Foot Care Nurse.

I am part of the PCN RN team at Battle River Medical Clinic in Ponoka. Chronic Disease Management is my role, and working with other partners in the clinic: physicians, nurses, dietician, foot care nurse, social worker and other supports, we want to be your medical home. In addition to supporting chronic disease, such as COPD, Asthma, Heart Disease, Kidney Disease, and Hypertension, a particular passion and interest of mine is Diabetes. I enjoy teaching, supporting and counselling the pre-diabetic, and diabetic patient. Working with you to set goals, providing education, checking in, and encouraging your positive changes are ways I can help you achieve your best health.



Leigh, Family Nurse, Sylvan Lake

As a family nurse I am part of a team focusing on health promotion and prevention. We provide guidance, education, and support in the management of various health conditions such as high blood pressure, type 2 diabetes, high cholesterol, COPD and asthma. My focus is to assist you with setting goals and developing plans that will help you to improve and maintain your health. I am also there to provide support as you move through various life stages from a first prenatal visit to end of life care discussions offering resources and encouragement along the way.

My role in the clinic involves supporting the physicians with patient procedures such as injections, suture removal, and dressings, as well as updating patient charts including medications, surgical history, and family history. My role also involves outreach and opportunistic screening for preventative measures such as routine bloodwork, mammograms and physicals, as well as identifying, managing, and maintaining patient panels for the physicians in the clinic.

My most embarrassing fashion trend I use to rock would have to be the wide bell bottom jeans.

My job involves primary preventative care for patients that I'm responsible for. I also support other PCN staff in screening for cognition and frailty. I work with a group of professionals to improve the health of my patients.

If the zombie apocalypse was coming I would want my husband as he can do pretty much anything, my brother-in-law because he can make anything taste fabulous, and my daughter as she can sew me up when I get hurt.

If I was a late-night talk show host my first guests would be some of the nurses I have had the privilege and honour to work with as they are amazing at their job but also amazing human beings!

Corinna, Panel Manager, Innisfail

My role at the Associate Clinic in Innisfail includes obtaining information for patient charts including: family history, past surgical procedures, and updating medication lists in the Patient Electronic Medical Record. I also track any of the Alberta Cancer Screening that patients may be due for and arrange for testing in the form of an appointment with their family physician or by completing a requisition for them to have their testing completed at the lab. These include: routine blood work, mammograms, colon cancer screening, cervical cancer screening, and setting up 24-hour blood pressure monitoring. I help to support the doctors at the clinic through opportunistic screening whenever possible and obtaining heights, weights, and blood pressures on patients. You can also find me helping out with answering phone calls and assisting with appointments when necessary. The Wolf Creek Primary Care Network has a variety of supports and programming available for patients to access through their family physician at the Associate Clinic and I help to support these referrals and ongoing information when required. Another form of screening that I am responsible for includes geriatric screening and frailty screening and education around goals of care and having a personal directive. I look forward to meeting you should you come into the Associate Clinic to see our Doctors: Dr. Barker, Dr. Rees and Dr. Maree.

My name is Jill and I have been a Licensed Practical Nurse since 2001. I am employed as a Panel Manager with the Wolf Creek Primary Care Network. I absolutely love this role as I get to know my patients while helping them navigate through the large ocean that is health care. I also love my team at Mainstreet Medical. They truly put the patient first.

What's the most embarrassing fashion trend you used to rock?

One time and one time only in grade seven I wore my jeans backwards (like Kriss Kross).

Who was your childhood actor/actress crush?

My actor crush was Brad Pitt. Now it's Uhtred from Last Kingdom.

My role as panel manager: I am responsible for four doctors' panels. I collaborate with these four doctors to ensure their patient panel is maintained with prevention and routine screening measures according to current guidelines. With direction from the physicians I contact patients with results or any information they need me to pass on to them.

If you were famous what would you be famous for?

I would be famous for singing Happy Birthday in opera every time. My children and nieces love it (not).

Hi everyone, my name is Kathy. I am a Panel Manager for Family Medical. My role as a panel manager includes daily day sheets for each of my doctors, answering many phone calls re patients results and nursing info, BP, Wts , and many other nursing procedures, keeping panels up to date with patients info, and P.N, results, also sending patients for their screening when due. Just a few of the many roles at Family Medical.

If I was famous I would like to be remembered as setting up and funding schools for all children in Africa, for a better life from them.

I am truly blessed to have such an incredible and innovative team at Wolf Creek Primary Care Network, and believe our team is what has helped us become and stay successful in providing leading edge primary care.

I’d be most intrigued to interview Elon Musk because he dreams big, sets big goals, and moves forward making what seems impossible come to reality (space rockets landing back on earth – WOW!). We have such an innovative team at Wolf Creek Primary Care Network and would make great partners if Elon would like to collaborate on future healthcare improvements (hint hint)!

Zack Morris from ‘Saved By The Bell’ – I hear they are even doing a remake of the show soon so there must have been a lot of crushes back in the day!

Rebecca, Practice Facilitator, Innisfail, Sylvan Lake, and Delburne

My role as a practice facilitator for the WCPCN is a bit hard to explain but in a nutshell I inspire and facilitate positive change. I work with physicians, allied health professionals, and community organizations to identify gaps in service, highlight quality improvement opportunities, and evolve the great work that is already happening in our clinics and towns. This can range from creating pathways to prevent people with elevated sugars from developing diabetes to addressing mental health in new and innovative ways. There is so much passion and dedication to mind-body wellness in the WCPCN. I love working with this team!

What's the most embarrassing fashion trend you used to rock?

Oh I wore it all and am not embarrassed by any of it! Ain't no shame in my game!

As a Practice Facilitator for the WCPCN I support physicians, clinic managers, and allied health care professionals in five medical clinics to deliver high quality primary care services. Working together with clinic staff and community partners we strive to make improvements in quality and efficiencies to the patient's medical home. I help identify what services are needed in each community and bring resources together to offer sustainable patient centered care.

Jim Carey. He is hilarious and I am sure it would be very entertaining.

Abri, IT and Evaluation Facilitator

(As written by Rebecca, Practice Facilitator): Abri is the owner of MedFin Consulting and comes to us with Honours in B Comm Accounting. Abri is an integral member of our crew who works with team members, clinics, and physicians to standardize data collection and evaluation across our PCN. This reporting relates to screening, referrals, program outcomes, and patient appointments - it helps us measure our successes and identify areas for quality improvement. He is also our tech support. In short, we would not be able to perform at the level that we do without Abri.
