The Wolf Creek Primary Care Network (PCN) has been operating in Central
Alberta since May 2008. Wolf Creek PCN provides services for patients in the
communities of Lacombe, Innisfail, Sylvan Lake, Ponoka, Rimbey, and Bentley. There are 64
family physicians working alongside other health care professionals in twelve
different clinics. The Wolf Creek PCN strives towards providing patients with
greater access to primary care.
The Wolf Creek PCN uses a collaborative team approach while providing care
for patients. By hiring other health care providers in addition to your
physician we assist patients with extra counselling, guidance, and education
throughout their health care journey. PCNs help physicians by allowing them to
see patients more quickly as a portion of the care they provide is delegated to
other health care professionals. Consistent communication happens between your
physician and the health care providers on your "team". We are also proud to
manage 24-hour patient access to appropriate primary care services.
Wolf Creek is just one of approximately 41 PCNs operating throughout Alberta.
Each PCN provides local solutions to local problems. The funding for each PCN is
public therefore there are no costs for patients to use the services and
programs available.
